Thank you for registering in an University of St. Thomas Continuing Education Graduate Course. We appreciate your support as we continue to provide courses at IGS that are "A cut well above the rest."
Important: Do not submit this "Request for Transcript Form" until after you have received your Grade Report from the University of St. Thomas.
If you send this request in before you receive the Grade Report not only will you not receive a transcript and loose the $5.00 processing fee, it will only delay the receipt of your official transcript even more.
Please help the University of St. Thomas transcript office serve you better by following the steps outlined above and on the reverse side.

All Transcripts provided will be the official university transcripts. Include a completed Transcript Request Form (attached):

Signature making the request

Please enclose a check or money order for the required $5.00 fee. If payment does not accompany your order, you will be billed $5.00 per copy. UST does not accept credit cards.
